For people switching to rosiglitazone and metformin from just metformin (Fortamet, Glucophage, Glucophage XR, Glumetza, or Riomet), the dose depends on the current metformin dose. For people taking 1000 mg of metformin total per day, the recommended starting dose of rosiglitazone and metformin is 2 mg/500 mg twice daily. For people taking 2000 mg of metformin total per day, the recommended starting dose is 2 mg/1000 mg twice daily. Your healthcare provider may increase your dose slowly up to 8 mg/2000 mg total per day if needed.
 

Rosiglitazone and Metformin Dosage for People Switching From Avandia

For people switching to rosiglitazone and metformin from Avandia (rosiglitazone), the dose depends on the current Avandia dose. For people taking a total of Avandia 4 mg per day, the recommended starting dose of rosiglitazone and metformin is 2 mg/500 mg twice daily. For people taking a total of Avandia 8 mg per day, the recommended starting dose is 4 mg/ 500 mg twice daily. Your healthcare provider may slowly increase your dose (up to 8 mg/2000 mg total per day) if necessary.
 

General Information on Dosing With Rosiglitazone and Metformin

Some considerations for people taking this drug include:
 
  • It is best to try to take this medication with food in order to avoid stomach upset.
     
  • Rosiglitazone and metformin should be taken at the same time(s) each day to maintain an even level of the diabetes medication in your blood.
     
  • If you do not take your rosiglitazone and metformin as scheduled, take your next dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue as scheduled. Do not take a double dose of this drug.
     
  • For rosiglitazone and metformin to work properly, you have to take it as prescribed. It will not work if you take it differently than your healthcare provider prescribes.
